Most insurers require policyholders to follow their administrative rules before they’ll pay the full amount outlined in the plan. For example, some plans require you to get a referral or pre-authorization before you receive treatment. Others require you to meet your yearly deductible before they’ll pay for some services.

Even if a specific treatment is covered by your insurance policy, you need to follow all the rules to get the full or partial reimbursement your plan entitles you to. If you forget a step, you could wind up with a big unexpected bill.
